The exact structure
All numbered points of the Annex VIII model — declaration number, identification of the packaging, manufacturer and Article 17 representative, object of the declaration, conformity statement against Articles 5 to 12, standards, signature block.
Guidance notes per field
Each field comes with a plain-language note: who draws the declaration up, one declaration per packaging type, how to reference the technical file, how long you must keep it — five years, ten for reusable packaging — and which languages are required.
Nine EU languages
The template is rendered in the language of the page you are on — the declaration must be translated into the languages required by each Member State where the packaging is placed on the market.
Version 2 — 29 October 2026. Re-read against the Official Journal text and the Commission guidance of June 2026: the conformity statement now follows Article 39(1) and cites Articles 5 to 12, one declaration covers one packaging type, and a table lists each requirement with the date it applies from.
